If GSM had been in place about the time CDMA came out, GSM would be on the same path in coverage in the USA, but the wireless industry didn't officially back GSM until around 2002 when nearly all TDMA carriers started converting to GSM, except for US Cellular, which converted to CDMA. Voicestream (maybe they were under another name before that) brought GSM to the states and was the only service using it in the 90's. I know because my friend in Lansing Illinois signed up with Voicestream in 1998 or 1999 and had a Motorola phone with GSM 1900 & AMPS 800.
