All the above points to the usual incorrect method of creating the CD from the ISO. I've lost track how many times folk create a bootable CD and place the .ISO on the CD.
Try creating a CD from the .ISO and it should boot. Resetting a password with NTPASSWD is simple with answering one question and yes to the next ones. This is why I don't write how to use it.
