Permissions repair in Disk Utility?

When I do a repair in the permissions part of Disk Utility, there is a list of mismatches that supposedly have been repaired. (Or so the line at the bottom says.) Even so, unlike what I used to see, so long ago, in Norton Utilities, if I run the permissions check after that, mismatches appear. Shouldn't there be no or few mismatches after a permissions repair? In Norton Utilities you got fewer and fewer (or no) problems showing up after a repair, and if you were persistent, you could end with none.
Am I just not getting what it is that permissions repair is supposed to do? I asked an Apple "genius" about this, and he waved it away, saying that it was of no consequence, that the permissions were repaired, no matter what it said on the screen.
My G4 Sawtooth desktop (with Tiger) and Macbook laptop (running Leopard) work well, although the laptop has a persistent though only occasional spinning beach ball and seems to have slowed down. I've been to one of the afore-mentioned "geniuses," but that didn't totally resolve that problem. I do have a new battery, which solved a different problem.
Thanks for your ideas about disk permissions, its function and whether what I see on the screen has any relationship to whether or not the permissions mismatches have actually been addressed.
