using PHP to store data in a MySQL-database. See if your ISP offers these tools.
You might find it helpful to install them (and a webserver) on your own PC also, because it's easier and faster to test locally. But that's not necessary.

Then start learning PHP and MySQL, buy books or follow a course and make what you need. My students mange to get a basic working application in 8 weeks, provided one of the group of 4 has some relevant experience.

You might be able to find code examples or even a completely working application if you start searching. But still, it's up to ypu to implement it on your site.

tectite's form. I've been using it for a few months now and it has many great features like custom templates and ways to "hide" your email address from the public view, which is important to keep your email account clear of spam. It's also very easy implement server-side validation if you require.
