The problem with Tennessee is that all the big insurers pulled out of the system. That is not possible under the Health Care act passed in Obama's first term. Tennessee bears no relation or resemblance to the Health Care Legislation as currently enacted, which is why you have never heard of it before.

The point of having a single payer system covering everyone, as is the case in Canada and Japan and Europe generally, is to use the economy of scale to reduce overall cost. The Affordable Health Care Act isn't set up that way, it allows the greedy and predatory private for-profit insurance companies to continue as they did before. That's one of the reasons I don't like the plan that was passed. That's also why the Tennessee situation is the way it is, because there isn't a central clearing house for payment which would be able to spot fraud more easily.

The Canadian system is organized by Province, but is the result of Federal legislation. That means that all the Provincial plans have reciprocal agreements and offer essentially the same services, and are managed by Provincial Civil Servants. There is no way the Civil Servants can funnel money to themselves, or to Doctors or Medical Companies or any other such idiocy.
