Proper use of the Option Button Macro

Hi, I am currently trying to generate a questionnaire in Word 2003, where people have to rate statements on a scale of 1 to 5. What I want is to show the statement and then below that, have a row of 5 buttons (1 to 5) they can click on.
I have tried getting it right by either using a new optoin button per row and copying it 4 times, or by creating a new button for every item (5 per row). However, no matter what I do, when I run it, I can always only tag one button. When I click on another button in the file, the old input gets erased.
Unfortunately, I don't know visual basic, so if anybody could help me out here it would be much appreciated.
