http://ad.velmedia.net is causing

When I look at the history I see 100's of references to http://ad.velmedia.net. I am guessing it is some sort of program that inserts ads onto/into the pages I am looking at whilst browsing. How can I eliminate it?

I'd suggest the AdBlock Plus Add-On.

NoScript is very helpful, too.

I would also recomend both of thoes add ons. I have not seen an add online in years!!!!!!
