Frgmented CD

I am new to itunes and have been importing some of my CD's into a new computer.
Most import just fine and list the CD title, then all the songs over to the side in the index.
My problem is that so far I have had two CD's that when imported are fragmented and is listed in multiple locations for some reason with a few songs in one spot and some others in another, sometimes three or four different locations for the same CD.

Is there anyway to prevent this and have all the songs for one CD to always appear properly under the CD heading?
