Unable to specify file name, location in save as dialog box

About a month ago, the "file name" and "save as type" fields, as well as the folder tree that usually appears on the left hand side of "save as" dialog boxes stopped appearing on my computer. This limits my ability to name and select location for new saved files. This happens in Microsoft Office 2007 and in Firefox Mozilla browser. I have a Windows 7 operating system. I would greatly appreciate your help in restoring those fields to the dialog box.
