can't believe i found this thread. i think i found a fix!
i literally spent my entire Sunday figuring this out. just bought a yamaha rx-v471 digital receiver and wanted to connect my windows 7 laptop to it. ran into all the problems above before finding an almost ideal solution that may help some of you.
after much trial and error, i got this to work by running an hdmi-out cable from my laptop to an hdmi-input on my hdtv. now my hdtv was already connected to my receiver via an hdmi-out (from the reciever) and also a digital optical audio cable (to the receiver from my hdtv). so i'm getting my iTunes library to play via my home theater speakers that are connected to the reciever - great success.
but on my receiver, the connection for some reason isn't attributed to one of my hdmi inputs. instead, it's attributed to one of my regular a/v inputs. i just have to change the input source on my receiver to one of the a/v inputs once the hdmi connection w/my hdtv is plugged in. not sure if i'm getting full digital audio but it has to be better than going analog w/rca plugs, right?
