Here is what the app info is under:

Samsung Remote
Samsung Electronics Co.LTD.
The support page from Apple is: http://www.samsung.com/us/#latest-home (This link has NO reference to the APPS team, just a bunch of mobile devices) Clicking on support from Samsung, sends you to a hardware product list where you get chat or a phone number to call... I had that experience.
This looks like Samsung to me that did the development.
I have identical symptoms for
3D Blu-ray Disc™ Player (BD-D5500) Model #: BD-D5500/ZA
60" Class (60.0" Diag.) LED 7000 Series Smart TV Model #: UN60D7000VFXZA
All Latest TV/Blue Ray Firmware and all apps up to date
First, the workaround, SMELLS like a software BUG. Why would it work the first time continuously, until you exit the app. This tells me the network is working fine, and protocols are in place. Someone needs to look at the code that manages connection states and find out why first time is working, and second time fails.
Second: The argument that this is not a Samsung network is ridiculous. Its like saying I can't fix your FORD because you have Michelin tires on it. I am running a Linksys Small Medium Business network at home (Not a low end network), ALL 1GE switching and routing interfaces. All LOCAL NAT, LOCAL IP Address. If Samsung cannot support the largest home router company in the world, you don't have a product. Larger companies (like the one I work for) WILL debug the problem even if it is not our gear and at least REMOVE the parent company from the issue and definitively tell the customer if it is in our equipment or NOT. We cant fix 3rd party (Unless we have a 3rd party agreement), but we can certainly point customers to them with the debug details.
In this network, there is NO Internet in this picture, it is a wireless Ipad connection to a Wireless router (gateway/internet capable) and the TV is HARDWIRED to the port on the router. So routing is 1 hop (Wireless IF to LAN Port). I have CAT 6 shielded cable in the whole house. Cant get much better than that. Distance to router, less than 30 feet. full BARS.
I looked up this Samsung company that did the app, and here is what I found:
Looks like the app was done by Korea corporate, and the TV/Blue ray Player is in the USA Wholly owned subsidiary.
I would expect Samsung to ignore looking at the network and focus on the firmware connection interface code and find out why this has been going on for over 6 months in this Cnet post with NO RESOLUTION.
This behavior, is identical for the Blue Ray Player as well. I just tried the removal of the I pad in the settings, and it works until you try it a second time.
Again, I will rate this app at 1. Also, looking at current stats, over 50% have rated it with a 1.

Great app but where is the keyboard? It does not appear automatically with You Tube. Please fix this.
The app won't hold a connection!

There has to be a bug on the app as it always disconnects from the
TV. The app can't hold a connection for more than 10 seconds.
