Samsung Rebate Status (> 1 year outstanding). Where to turn?

Dear Samsung and loyal customer.
I'm really not sure where to turn to here as I have tried for over a year to no avail.
I purchase a Samsung LN46A850 in Nov of 2008, and it qualified for a $300 rebate. I love the TV, still do, but to this day I have never received the rebate in the mail.
This wasn't due to the fact that it wasn't authorized or approved, I have a statement on (checkmyrebate.com) stated it was approved, but it has been pending payment for 1+ years. I had received a few responses from your 3rd party regarding payment status (awhile ago), and I attempted to get a hold of them for months. I've since almost given up. As I have used this forum in the past (to find other useful information), this is my last ditch effort to get some help.
I've included my information below.
Any help from the Samsung staff here would be most definitely appreciated. Again, it wasn't that the 3rd party fulfillment didn't approve the rebate, it's that it was held up (probably misplaced) for shipment, and at this point the 3rd party will not return calls or e-mails.
