Database? Data mining? Software? Webware? I am not to sure.

I would like to be able to:
Have a web based interface for clients to enter detailed typed information and to choose from a group of radial buttons in a questionnaire.
Then for that information to be stored in a database and for the data to be mined in a way that their answers and keywords they type categorizes their response. And that information generates a report.
For example:
I love to ride my motorcycle fast. I fight with my mother. I have a fear of spiders.
Keywords:
motorcycle + fast = freedom
fight + mother = power
fear + spiders = control
and also, when they click on the radial buttons, if they answer yes to 80% of questions in Group A and 30% in Group B, and 70% in Group C, then they are grouped into Group X.
Is there any such software or webware that does this and can be easily tailored to meet my needs?
