Vista Family Discount Problems

I just got Vista Ulimate and wanted to use the family discount yet the order form does not seem to work anyone else having this problem to?

I live in Hawaii, just bought Vista Ultimate and am not able to access/fill out form to order the discounted 2 keys for the Home Premium up grade at the website. Is there a customer service phone number that I can use to call for help?
