It's a few $$$, but iphoneringtonemaker (www.efksoft.com/products/iphoneringtonemaker/index.php) works very well.

I think that the new version of iTunes does this now for free though.

ringtone designer

I use Ringtone Designer. It will only work with purchased itunes songs.
