Are there any laptop coolers out there that really make a difference?
I use these mostly on those old Pentium 4 laptops. Those designs appear to be for short time use as the machine temperature climbs and it crashes even after we tear it down to clean all the dust, lint out then put in fresh heatsink compound and pay attention that the heatsinks are seated proper and put it all together.
Figure that many will not want to risk such an adventure they should at least be doing the yearly blast of air in the vents and augment the cooling with the above units.

solutions few and largely ineffective. Be very careful adding hardware on a Laptop. Add a few degrees and crash!
I clean my PC every three months and change thermal paste once a year in the Spring.
This software can be used to take your Laptop's temperature. Check it while doing a virus scan.

I am very happy with the Targus X rack, as it's lightweight, portable, doesn't need to be plugged in and holds the laptop up off of the desk, your lap, your bed, the sofa, etc. The mfr link is: http://www.targus.com/US/product_details.asp?SKU=AWE09US but you can find it way cheaper if you look around. I bought mine on sale at one of those daily deal plces, but this gives you an idea of what it looks like. When I was shopping for one, I researched different kinds and found that the ones with fans burn out in time! This is non mechanical and just folds up and stores easily in your laptop case. Only 5.6 oz too.
I have been using an akasa cooler for the last two years and have been very pleased with the job it does. It connects via a usb port on the laptop for power to its two reversable fans but also includes two extra usb ports. The approx. ten degree top angle makes for a comfortable working position. I purchased this in the UK from Misco (misco.co.uk) and I believe it is still available.
Stargazer--If desks and tables offer sufficient cooling and they do, all you need is a table that fits on your lap and has no legs. Get a wooden sandwich board the size of your computer. I have one that just fits my 15.4-inch screen laptop. It cost roughly $10 at the local supermarket. It also fits inside my laptop's carrying case along with my laptop--in the same slot, protecting it from hard knocks from the outside. AFvW
I have a SIIG and it works great. It adjusts three or four heights and swivels and it does a great job at keeping my laptop cool.I think I paid about 23 or 25 dollars for it.Just plug it's usb into the back of your laptop and you're ready to rock and roll lol.I have had it for a few months and I can't remember the last time my pc crashed.Good luck.
